Forms
ICOS Forms is a feature that allows casinos to create Electronic Forms that would otherwise exist as paper based forms or electronic documents such as Word or Excel.
1. Creating an Electronic Form
Below is the initial form creation screen. The ICOS manual provides more information on creating electronic forms.
2. How to use the Electronic Form
Once the Electronic Form has been created, it can be associated with one or more incident/occurrence types. When an incident/occurrence is logged, the user clicks on the “Further Details” button to access the form (see below).

The form below is an example of a “Baccarat Table Audit” form created by a casino.
3. Advantages of Electronic Forms:
- Electronic Forms allow users to search for information based on certain criteria that is extremely difficult, if not impossible when using paper or electronic documents e.g. audits or reviews conducted on or by particular person, audit or reviews for a particular date range etc
- ICOS Electronic Forms provide “Collusion Reporting” features
- ICOS Electronic Forms can form part of a work-flow process e.g. when a review is done that requires feedback from a department head, the department head will be notified automatically of a breach in procedure and be required to provide feedback by a certain time-frame.
- Electronic Forms are stored in a secure electronic database.
